  • You must experience chronic heart failure while on a regimen of prescribed treatment
 
There are many prescription medications used to treat chronic heart failure. In order to qualify for benefits, you must continue to experience heart failure even while on medication. Some common medications include Beta blockers and ACE inhibitors.

  • You must have medical evidence of systolic or diastolic failure
 
Systolic failure means that your heart can’t contract normally and pump out enough blood. Diastolic failure, on the other hand, means that your heart can’t relax normally and fill with enough blood. You only have to experience one of these two types of failure to qualify.

Medical evidence includes appropriate X-rays or other forms of imaging. Your doctor should know which type of images are necessary for your condition. Your medical history may also be used to determine if your heart failure is chronic.

  • You must experience one of the following symptoms or side effects
 
Chronic heart failure can affect your ability to function in many ways. To qualify, you must experience one of the following limitations.

  • General inability to complete daily activities due to heart failure symptoms. A medical consultant must also determine that you cannot safely perform an exercise test.
  • Three or more episodes of heart failure within 12 months. These episodes must be acute and require medical assistance.
  • Inability to perform an exercise tolerance test at 5 METs or less. 1 MET is the amount of oxygen you take in while resting.
